Time: 1:15-2:30 p.m. or 6:00-7:00 p.m.
Please bring to registration:
1. A copy of the birth certificate and baptismal certificate
2. A copy of the child’s Social Security card
3. A copy of immunization records
4. A $50 nonrefundable fee
